Your Rights and Protections Against Surprise Medical Bills
Notice of Policies and Practices to Protect the Privacy of Your Health Information
Payment is due at the time of service. If clients do not make payments within 60 days, their account balance is considered “past due.” If this occurs, Provider will ask clients to set up payment plans. If account balances continue to be past due for 90 days or longer, a fee of $50.00 will be applied to the account. If account balances remain unpaid after 120 days, accounts will be passed to a collection agency.
Fees
I am not in-network with any insurance companies. Fees are self-pay only. Payments accepted include HSA, FSA, and all major credit cards. I can provide you with billing statements if you would like to use out-of-network benefits. Please contact your insurance provider in advance to confirm your out-of-network policy.

Cancellations and Missed Appointments
If you need to cancel or reschedule your appointment, please do so by email or voicemail at least 24 hours in advance of your appointment. The full session fee is charged for missed appointments or appointments cancelled with less than 24 hours notice.
